Ellen Vance’s testimony
I received Christ as my Savior when I was nine years old, but it wasn’t until I was thirteen that I truly surrendered
my life to the Lord. My brother had passed away in a car wreck while under the influence of drugs and alcohol.
That tragic event made me think about my life and brought Heaven and Hell into focus. I surrendered my life to
Christ in August of 1977 and began a personal relationship with Christ. I spent my last two years of high school at
Hazard Christian Academy and graduated in May 1980.
My life was like most, met a boy, got married and began a family. I have been so blessed to be a mother to three
of the most beautiful souls. My marriage of 20 years ended because my husband was not faithful and left us for
another woman. I went through a lot of years as a single mother raising three girls while graduating college. My
relationship with Christ got me through a lot of tough years. Those ladies gave me five of the most loving,
beautiful grandbabies to cherish.
My life has had many tears and sorrows. In March 2025, my husband went to be with the Lord after a battle with
Pancreatic Cancer. It’s easy to question God when tragedy strikes, but I learned to depend upon the Lord in every
situation. Through the bad times and the good, God is always faithful!
The experiences I gained throughout my working career has led me to where I am today. I was working two jobs
trying to make ends meet when I heard of an opening at Kentucky Mountain Health Alliance, Inc., Little Flower
Clinic. I began as an assistant to the CEO. After two years the CEO decided to resign and move out of state. The
board of directors did a search for candidates to take her position. Three candidates were chosen and
interviewed. The board wasn’t satisfied with either of the candidates and approached me to take the position. I
became the CEO in May of 2015. I have been blessed to see our clinic grow over the years and I know that God has
his hand on our organization. KMHA has a lot of people who possess a servants heart.
I love the mission and vision of KMHA. I love helping people reach their full potential. I love giving back to a
community that helped take care of me when I needed them the most.

I met Ellen on my first trip to Hazard, Kentucky in April 2025 Visited again a few weeks ago. Great soup kitchen-outreach to the homeless in Hazard, Kentucky. Ellen is a very committed passionate lady. Loves her work!

KENTUCKY MOUNTAIN HEALTH ALLIANCE, INC.
The Executive Director is directly employed and reports directly to the Board of Directors and is responsible for
the management of the implementation of grant programs designed to increase the quality of life for the
community and surrounding areas; responsible for planning, implementing, and evaluating services. The
Executive Director oversees other key management staff in carrying out the day-to-day activities necessary to
carry out the proposed project.
Clinic established in 2005
Kentucky Mountain Health Alliance, Inc. (KMHA) was established in 2005, in the basement of a Catholic Church, by a group of
community healthcare and social service partners who had a similar mission to create a healthcare safety net for the
underserved or at-risk for homelessness population. KMHA is proactive in our effort to protect the health and the delivery of
essential human services to those experiencing homelessness or those at risk, especially for those who are least able to help
themselves. Kentucky Mountain Health Alliance, Inc, (KMHA) is funded through HRSA/Bureau of Primary Health Care, Health
Care for the Homeless, GRANT # H80CS07772. The mission of KMHA is to improve, in a respectful manner through a
coordination of quality holistic services, the health and well-being of Southeastern Kentucky citizens who are underserved or at
risk for homelessness. The vision of KMHA is to ensure access for all people and to eliminate disparities through provision of
quality holistic care in an integrated system of services for the citizens of Southeastern Kentucky.
Perry County is located in the heart of the Appalachian Coalfields. Both the county and county seat, Hazard, Kentucky, are
known for their historical connections to the War of 1812, both being named for Commodore Oliver Hazard Perry, a naval hero
in the War of 1812. According to the US Census Bureau, the county has a total of 343 square miles of which 0.9% is water. Perry
County is rural, with nearly 74% of the population living in a low population density area (500 or fewer people per square mile
and less than 2,500 people).
Two Clinics and a Mobile Medical Unit
Kentucky Mountain Health Alliance, Inc., operates two clinics and a mobile medical unit. Little Flower Clinic was the first clinic
established in 2005. Little Flower Clinic hours are 7:00 a.m. to 5:00 p.m., Monday through Friday. KMHA Quantum Healthcare
was opened in July 2023 as part of our healthcare operations. Quantum hours are 7:00 a.m. to 11:00 p.m. Monday through
Friday and 10:00 a.m. to 11:00 p.m. Saturday and Sunday. Both clinics offer an array of services including but not limited to,
Medical, Dental, Pediatrics, Women’s Health, X-ray and Ultrasound Department, Fully operating CLIA Lab, Behavioral Health,
Youth Prevention, Wrap Around Recovery Program, Chiropractic, Rheumatology, Substance Use Disorder, Medication Assisted
Treatment (MAT), Chronic Disease Case Management, Social Determinants of Health Case Management (Food Insecurities,
Transportation Barriers, Emergency Medications, Crisis Aid Program, Insurance Eligibility, Kentucky Prescription Assistance
Program), 340b Drug Program, Sliding Fee Program and 35% Discount if paid on Date of Service. KMHA affirms that no patient
will be denied services because of inability to pay. Patients will be expected to comply with the efforts of the Front Office Staff
to ascertain the existence of any third-party insurance coverage a patient may possess, and/or to exhaust all other payment
sources, or otherwise appropriately document the patient’s ability to pay for services. Discounts are offered to uninsured and
underinsured patients based upon family/household size and annual income. Sliding scale discounts will be based on the most
recent Federal Poverty Index guidelines.
